It is $10 for every in-vitro diagnostic medical device.<\/li>\n<li>For Class C or Class D medical devices, apart from in-vitro diagnostic medical devices, the fee is $3,000 for a single manufacturing facility and $1,500 for every medical device.<\/li>\n<li>For Class C or Class D in-vitro diagnostic devices, the fee is $3,000 for a single manufacturing site.
